New oral antidiabetic drugs
improve glycemic control but…

Vulvovaginitis, balanitis and related genital infections are common in patients with type 2 diabetes mellitus. Possible causal factors include glycosuria, which is the result of treatment with oral antidiabetic medication (also improperly called oral hypoglycaemic drugs).

Such drugs, used to treat type 2 diabetes, belong to various classes and reduce glycaemia, with mechanisms that are sometimes very different, for example by increasing the renal elimination of glucose.

This is the case of SGLT-2 renal glucose transporter inhibitors, which partly reduce the reabsorption of sugar filtered by the kidneys, thus lowering glycaemia due to an increase in glycosuria (excretion of glucose in urine).

Such mechanism of action increases the frequency of genital and urinary tract infections (cystitis), as confirmed by safety data from 12 randomised and placebo-controlled studies, grouped together to assess the correlation between glycosuria and infections in patients with type 2 diabetes mellitus. From the studies it emerged that treatment with SGLT-2 renal glucose transporter inhibitors is associated with a greater risk of vulvovaginitis, balanitis and urinary tract infections.
